Business Card Orders

  • Each Provision Living community has a customized version of a business card, reflecting their brand standards.
  • A Department Head and Executive Director should have a business card with their full name, title, any credentials, and contact information.
  • For cost purposes, we print in QTYs of 500 or 1000.
  • Human Resources will review each request to fact-check title and name.
  • Each business card request will be emailed to the person requesting the card to verify that all information is correct and accurate. To maintain timeliness of production, we ask for a reply with in 24hours>> any longer can result in delays.
  • Typical delivery time is 5-7 business days from the approval date. Not the submission date.
  • When requesting a card with a cell phone number, please confirm with Regional Director or Vice President of Operations that there is a cell phone agreement in place for the person on the requested card. Business cards cannot be produced with out this agreement in place.

ReOrders can be ordered from The InkSpot. This is for changing a current business card or creating a new one.

You may request business cards at any time, but please make sure to request early enough to allow time for production and delivery. Typically 5-7 business days from request.

  • Who is this business card for? Include any credentials (RN/LPTA, BSBM, MBA/HCM, etc.)
  • Please confirm with Regional Director or Vice President of Operations that there is a cell phone agreement in place for the person on the requested card. Enter 0's if not applicable.
  • Please add (000)000-0000 if you do not have a fax line.
  • Enter the community address and email address you want to appear on the business card.
    Typically, we order 1,000 business cards for ED, CRD and DON positions. 500 for all other department heads.
